Dann liegt die Antwort vielleicht au\u00dferhalb des Genoms. Genau hier setzt CeGaTs ExomeXtra\u00ae an: Es kombiniert eine umfassende genetische Analyse mit dem Nachweis relevanter Virusinfektionen. Ein aktueller klinischer Fall zeigt, wie wirkungsvoll dies ist.<\/p>\n<p><strong>Ein Fall ohne klare Ursache &#8211; vorerst<\/strong><\/p>\n<p>Ein 35-j\u00e4hriger m\u00e4nnlicher Patient stellte sich mit thrombotischer Mikroangiopathie (TMA) und akutem Nierenversagen (Stadium 5) vor. Die Diagnose einer TMA wurde durch die histologische Untersuchung einer Nierenbiopsie best\u00e4tigt. Daraufhin wurde bei CeGaT eine genetische Analyse mittels ExomeXtra\u00ae durchgef\u00fchrt. Anschlie\u00dfend wurden alle ph\u00e4notypisch relevanten Gene analysiert, einschlie\u00dflich genomweiter Kopienzahlvarianten (CNVs), die jedoch keine Ursache f\u00fcr die Erkrankung ergaben. Stattdessen zeigte sich ein auff\u00e4lliger und klinisch relevanter Hinweis: In der Patientenprobe konnten zahlreiche Kopien des Parvovirus-B19-Genoms nachgewiesen werden \u2013 ein eindeutiges Zeichen f\u00fcr eine aktive Parvovirus-Infektion.<\/p>\n<p><strong>Parvovirus B19 dank ExomeXtra\u00ae identifiziert<\/strong><\/p>\n<p>Dieses virologische Ergebnis lieferte einen entscheidenden diagnostischen Hinweis: Das Parvovirus B19 wird in seltenen F\u00e4llen mit TMA in Verbindung gebracht, auch bei immunkompetenten Erwachsenen, wie bereits in der Fachliteratur beschrieben<sup>1<\/sup>. Auf der Grundlage dieser Erkenntnis wurde die Behandlung des Patienten angepasst, um die Virusinfektion zu bek\u00e4mpfen. Das unterstreicht die unmittelbare klinische Relevanz des Ergebnisses.<\/p>\n<p><strong>ExomeXtra\u00ae erkennt, was andere \u00fcbersehen<\/strong><\/p>\n<p>CeGaTs ExomeXtra\u00ae erweitert die Exom-Diagnostik um das Screening auf aktive Infektionen. Dazu geh\u00f6ren Krankheitserreger wie Toxoplasma gondii, Varizella-Zoster-Virus, Cytomegalovirus, Parvovirus B19, Treponema pallidum, Epstein-Barr-Virus und Herpes-simplex-Virus Typ 1 und 2. Urspr\u00fcnglich f\u00fcr die pr\u00e4natale Diagnostik entwickelt, hat sich dieser erweiterte Ansatz inzwischen auch dar\u00fcber hinaus bew\u00e4hrt. Der vorliegende Fall zeigt deutlich: Der Nachweis einer Virusinfektion \u2013 in diesem Fall des Parvovirus B19 \u2013 kann die klinische Entscheidungsfindung ma\u00dfgeblich beeinflussen, selbst bei erwachsenen Patientinnen und Patienten.
